You next step will consist of an examination and consultation explaining your diagnosis and treatment options. In many cases root canal therapy can be completed at this appointment. Surgery and/or a complex medical history and treatment planning may require an evaluation and a second appointment for treatment.

Please assist us by providing the following information at the time of your appointment:

  • Your treatment referral slip and any x-rays if applicable
  • A list of medications you are presently taking

If you have dental insurance, bring your current insurance card. If you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.

IMPORTANT:  All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. uncontrolled diabetes, high blood pressure, artificial heart valves and joints, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays
If your dentist or physician has taken x-rays, please have them forwarded to our office as they may be helpful in our diagnosis. However, several additional X-rays will be taken by our office the day of your consultation and throughout treatment.
